- Curriculum vitae (CV)
- Graduated from an ACPE-accredited school of pharmacy
- Official transcripts from an ACPE-accredited school of pharmacy
- Eligible for licensure in the state of Maryland
- Three recommendations
- (Must have ONE from a Clinical Faculty Preceptor/Mentor)
