Applicants must meet the following criteria:

Have completed an ACGME accredited residency program by the time of fellowship

Applicants must be board certified in Internal Medicine, Family Medicine, Neurology, Psychiatry, Surgery, Emergency Medicine, Pediatrics, Radiology, or Ob/Gyn, or board eligible with demonstration of active pursuit of certification

A personal statement describing the applicant's interest in and commitment to a career in palliative medicine and hospice

Three letters of recommendation, one of which should be from the applicant's department head, program director, or division chief and at least one letter should be from a clinician in the field of hospice or palliative care
