Individualized geriatric patient-centered care

Partnering with patients, as well as their family members and caregivers, the Center for Successful Aging provides individualized treatment plans that evolve with the patient’s changing needs—whether they receive care at home, the hospital, or a long-term care facility.

The practice has been designed with a patient’s comfort and convenience in mind. The team focuses on the unique needs of older adults who are experiencing multiple chronic medical conditions, such as diabetes, high or low blood pressure, chronic obstructive pulmonary disease (COPD), vascular disease, heart disease, or arthritis. It also provides care for geriatric syndromes, such as frailty, incontinence, memory loss, vision and hearing changes, or problems with balance or mobility.

Social and support concerns are addressed, including isolation, difficulty affording care on a fixed income, transportation and housing challenges, food insecurity, or need for information and connection to resources in the community.

The Center for Successful Aging is a participant in the Age-Friendly Health Systems Initiative of the John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) – a multi-year initiative to improve how health systems provide care to older adults. Our person-centered care approach can help patients successfully age in all ways – physically and emotionally – while providing vital support to caregivers.

Addressing age-related issues

The Center for Successful Aging addresses the unique needs of people ages 65 and older who experience multiple medical conditions, geriatric syndromes, and the social needs associated with aging. It enables seniors to receive comprehensive, coordinated care of chronic and complex conditions all in one convenient location.

Our team provides care for a variety of common age-related conditions, including:

  • Dementia
  • Confusion or memory problems
  • Behavioral changes, including depression or anxiety
  • Chronic medical conditions, such as diabetes
  • Decreased mobility
  • Difficulty in performing daily activities
  • Frequent falls
  • Impaired vision or hearing
  • Managing multiple medications
  • Reoccurring or constant pain
  • Slower reaction times

In addition, we provide education, support, and encouragement for not only our patients but also for their caregivers and family.

What to expect

A first step is for patients and caregivers to complete a Life Plan Questionnaire, which can help connect them with resources to foster good health and independence for as long as possible. In other words, it helps patients “age their way.”

The Center for Successful Aging’s model of care is well aligned with the IHI Age-Friendly framework, focusing on four essential elements knows as the “4Ms”:

  • What Matters – What are your health goals? What is most important to you and what will increase your happiness?
  • Medication – Our teams help you and your caregivers organize and manage multiple medications.
  • Memory – Our experts work with you to develop the Life Plan in order to manage and treat medical and social concerns. New patients receive a comprehensive health assessment that identifies cognitive challenges and issues with depression, dementia and Alzheimer’s, and other challenges related to mood, behavior, thinking, and problem-solving.
  • Mobility – Our teams assess your strength, balance, and functional mobility to help you safely function and reduce the risk of falls and fractures.
