This one-year, evidence-based group support program is based on the National Diabetes Prevention Program, designed for individuals who are at risk to develop type 2 diabetes and those who have been diagnosed with pre-diabetes. It is also designed to help adults with high cholesterol, overweight/obesity, and families whose members have been diagnosed with diabetes. With the support of a lifestyle coach, participants are supported in this journey toward making positive changes related to nutrition, exercise, problem-solving, and coping skills.
