MedStar Health Epic MyChart – Terms and Conditions of Use
MedStar Health’s Epic MyChart (“MyChart”) is an Internet application that (i) enables patients to have secure web-based access to information from their medical record maintained by MedStar Health and its affiliated entities (collectively referred to as “MedStar Health”) in electronic format, (ii) provides patients and their guarantors the ability to pay for services; and (iii) provides a secure mechanism to communicate with providers. MedStar Health is offering this service to patients so patients have a convenient means of reviewing information from their medical record, communicating with their MedStar Health healthcare providers, arranging for clinical services and appointments, and paying for services rendered.
The electronic and accessible nature of medical record information maintained on MyChart presents unique security and privacy issues as addressed below. Therefore, certain necessary measures for protecting the security and privacy of such information are the responsibility of the user as detailed below. By requesting a MyChart account and continuing to use MyChart, you are agreeing to these Terms and Conditions of Use, which include legal and privacy waivers, disclaimers, and personal responsibility requirements.
The information from your medical record available through MyChart may not constitute your entire medical record. The information available through MyChart is limited to that information that is maintained within MedStar Health’s primary electronic medical record system (Epic). You may have records stored in other systems that cannot be viewed through MyChart. There also might be technical or legal limitations on what is viewable to you in MyChart. You will continue to have access to your full medical record by contacting the office of your health care provider directly or by submitting a request through MedStar Health’s medical records request process which can be found at https://www.medstarhealth.org/services/medical-records. You can also request a copy of your full medical record by submitting a request through the Sharing Hub section of your MyChart account. YOU ACKNOWLEDGE THAT MYCHART WILL NOT BE USED TO MAKE HEALTHCARE DECISIONS OR DIAGNOSIS AND MEDSTAR HEALTH WILL NOT BE LIABLE FOR ANY PERSONAL INJURY, INCLUDING DEATH, ARISING FROM YOUR USE OR MISUSE OF MYCHART OR ANY INFORMATION OR CONTENT THEREIN. Remedies under these Terms and Conditions of Use are sole and exclusive and are limited to those expressly provided for in these Terms and Conditions of Use.
Research Recruitment via MyChart
MyChart may be used to send invitations for research studies you may be eligible to participate in. You may be able to opt out of research recruitment messages through your MyChart communication preferences and/or by contacting the MyChart Help Desk.
Autmatic Paperless Billing Enrollment
Guarantors enrolling in MyChart on or after October 1, 2027 may be automatically enrolled in electronic statements (“paperless billing”). You may be able to opt out through your billing preferences in MyChart or by contacting MedStar Patient Financial Services.
General Terms
You are not required to utilize MyChart and may discontinue usage at any time. You acknowledge that you are expected to use MyChart appropriately. Overusing the messaging feature of MyChart or using inappropriate or threatening language are a violation of these Terms and Conditions. You acknowledge that MyChart is being provided to you without charge. Therefore, MedStar Health reserves the right to terminate your access to MyChart at any time, with or without cause. Should MedStar Health determine that you have violated these Terms and Conditions of Use, as may be modified from time to time, MedStar Health may discontinue your usage of MyChart immediately, without providing prior notice. In addition, MyChart may not be available at all times due to system maintenance, outages, or other issues beyond the control of MedStar Health. BY USING THIS SERVICE OR BY CLICKING “ACCEPT” BELOW, YOU SIGNIFY YOUR AGREEMENT TO THESE TERMS AND CONDITIONS OF USE. IF YOU DO NOT AGREE TO THESE TERMS AND CONDITIONS OF USE, DO NOT USE THIS SERVICE.
MyChart enables you to send requests, questions and other communications, regarding your medical record information, electronically to your health care providers. Such communications will be routed to an appropriate individual within your health care provider's department and/or practice. Therefore, individuals other than your personal health care provider may review your medical record information as required to address your request. If your communication contains sensitive information that you want shared only with your personal health care provider, you should contact such provider directly and not through MyChart. Communications sent through MyChart may become part of your permanent medical record and may be used and disclosed as permitted by law.
The average turnaround time for responding to requests for information submitted through MyChart is two to three business days. Therefore, communications via MyChart for urgent matters is not appropriate. In those circumstances, contact your health care provider’s office directly. In emergency situations, please dial 911. Information in MyChart is for informational purposes only and may not be updated in real time. Additionally, the information may be available to you before your healthcare provider has had the chance to review it. The information is not a substitute for professional medical advice and should not be used for self-diagnosis or self-treatment.
Communications from your health care providers via MyChart will contain important information regarding your health care. It also is possible your health care providers may decide not to communicate such information by other means if communicated through MyChart. Therefore, it is important for you to monitor regularly your message inbox in MyChart and your personal e-mail for message notifications. You agree that you will not hold MedStar Health liable for loss, injury or claims resulting from your failure to read messages in a timely manner.
You will be notified via e-mail when there is new medical information to be viewed on MyChart. Notifications may include, but are not limited to, provider messages, test results, prescription renewals, letters, questionnaires, after visit summaries and appointment status and reminders. Any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent via email, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. You should take this into account when providing an e-mail address and when sharing any information required to access your personal e-mail account. By accepting these Terms and Conditions of Use and continuing to use this site, you agree to receive communications by e-mail.
You will also be given the opportunity to access patient educational materials through MyChart. These educational materials are provided by a third party and for your convenience only. We may or may not have any control over these items or their content and do not assume any liability or responsibility for them. Any questions you may have about your care or treatment should be addressed directly with your providers, and you should not rely on these educational materials as your exclusive source of information.
MyChart Bedside
You may be offered MyChart Bedside to access some of your clinical information while you are admitted as an inpatient to a MedStar Health facility. MedStar Health is under no obligation to offer any patient the ability to use MyChart Bedside and may exercise its discretion in offering such use. Use of MyChart Bedside is free of charge to you as the patient. Some of the information in MyChart Bedside may only be available for a limited time after you are discharged, and not all past inpatient encounters will be available for viewing.
You may be offered the use of a MedStar Health-owned tablet or other device to use while an inpatient to access MyChart Bedside. You will be able to access and use several applications (“apps”) on the device using your own personal account log-in. You will not be able to download any apps that have not been preloaded onto the device. By agreeing to use MyChart Bedside, you understand that the device provided to you is a loan. You agree not to take the device outside of your hospital unit and agree to take reasonable measures to protect the device from loss or damage. You may be responsible for any loss or damage to the device caused by you. At the end of your hospitalization, you agree to return the device to the docking station or designated area within your hospital room or to a member of your clinical team. All of your personal health information and any other data, including any log-in information, that was stored in the device will be erased once you are discharged from the hospital.
If you do not have a MyChart account yet, you will be given the opportunity to create one during your hospitalization.
The information in MyChart Bedside may not fully reflect clinical data, such as the clinical staff assigned to your care team. If you have any questions, you should ask a clinical team member. MyChart Bedside should not be used as a replacement for necessary communication with your clinical care team. Any question you may have about your medical care or lab results should be directed to your care team. You should not rely on information provided by MyChart Bedside to make decisions.
MyChart Bedside has a feature called “Happening Soon”. You may enter personal events, but those will not be visible to the clinical staff involved in your care. Please note that entering this information is for your purposes only.
MedStar Health reserves the right to remove the device and/or discontinue your MyChart Bedside account or a portion thereof for any reason, including, but not limited to, abuse or mishandling of the application or device, interference with clinical care, behavioral issues, or failure to comply with these Terms and Conditions of Use or MedStar Health’s policies. Such termination may occur without any notice to you. These actions are in addition to and not in lieu or limitation of any other right or remedy we may have available at law. Further, we shall not be liable to you or any third party for any such termination or discontinuance. The preceding Terms and Conditions of Use will also apply to your use of MyChart Bedside. Any reference to MyChart in the following Terms and Conditions of Use will include MyChart Bedside.
MyChart Central
If you have MyChart account(s) at other organizations in addition to a MedStar Health MyChart account, you may be able to view all of your information from those organizations after logging into your MedStar Health MyChart account. This feature is called MyChart Central, which you may activate within your MedStar Health MyChart account under Profile, Link My Accounts. This feature allows data from multiple MyChart accounts to be viewed in one place, but does not affect the underlying data that resides within each of your separate MyChart accounts. Due to technical limitations of the system, it is possible this feature may not be available to you.
Security Considerations
Information from your medical record that is accessible through MyChart is intended for your access only (subject to the discussion below under “Proxy Access”). You will have a unique username and password for MyChart and, where applicable, a PIN for MyChart Bedside, which will provide you with such access and are crucial to protecting the security of such medical record information. Therefore, you should select a password or PIN that is confidential and it should not be shared with anyone. In the event you believe your password or PIN has been compromised, you may change it anytime through MyChart.
MedStar Health will utilize certain security measures within the scope of MyChart such as verification of identity through the use of specific activation codes and the tracking of any activity within the account. However, once you access your medical information through MyChart, you are solely responsible for maintaining the security of such information on your end as well as the information required to access such information (i.e., username and password or PIN). Specifically, you are responsible for the results or consequences of your communications regarding, or the sharing of, information accessed via MyChart.
In connection with your use of MyChart, you agree that MedStar Health may monitor your individual use and may collect data or information about your use of MyChart to use at its discretion. Information collected may include but not be limited to, for example, access dates and times, periods of connectivity, information relating to web pages visited, and other information relating to your access or use of the services.
Minors
An individual must be age 13 or older to request a personal MyChart account. Parents or legal guardians of minor children may request access to the minor child's medical information by following the terms outlined in the “Proxy Access” section of these Terms and Conditions of Use. However, the full spectrum of MyChart services will not be available for the records of minor children between the ages of 13 and 17 (inclusive). Under state and federal law, there are certain types of medical information that the parent or legal guardian of a minor patient may not view without consent of the minor patient. Because of these requirements, the full spectrum of services to parents or legal guardians of minor patients in this age category cannot be offered. Thus, when a minor patient reaches age 13, restrictions will be placed on parents’ or legal guardians’ MyChart access until the minor reaches age 18. At age 18, a child is no longer a minor and access by parents or legal guardians will be terminated automatically. If you are the legal representative of a patient, you may still be entitled to receive a copy of the patient’s medical record by contacting the provider directly.
Proxy Access
Proxy Access Based on Age
A parent or legal guardian is permitted to have access to his or her minor child’s online health information through MyChart. This type of access is known as proxy access. Until a child turns 13, a parent or legal guardian may have full access to their children’s online health information. Due to technical limitations and certain State and District of Columbia confidentiality laws that allow minors to consent to limited treatment services on their own and control access to the information related to those treatment services, a parent or legal guardian of children aged 13 to 17 years of age (or younger in the provider’s discretion with respect to certain services) has limited access to the minor’s online health information. The amount of information the parent or legal guardian will be able to see in MyChart does not affect any other rights the parent or legal guardian may have to obtain a minor child’s medical records through other mechanisms. Patients 13 or older are eligible to have their own MyChart account. At age 18, a patient is no longer a minor and proxy access by parents or legal guardians is automatically terminated.
Proxy Access for Adult Patients
Patients 18 years or older may authorize other individuals to have access to their online health information. The Sharing Hub in MyChart can be used to send invitations to give access to a proxy.
Proxy Access Based on Medical Status of Patient
Legal representatives of patients who lack the capacity to make their own health care decisions are permitted full proxy access to the patient’s online health information upon presentation of appropriate legal paperwork.
Once a patient is deceased, a proxy’s access to the patient’s MyChart account is valid for a period of thirty days and then access is automatically terminated. The personal representative of a deceased patient who is not currently a proxy may be permitted to request full proxy access to the deceased patient’s online health information upon presentation of appropriate legal paperwork and as permitted by law and MedStar Health policy.
If the proxy’s legal relationship with the patient changes, the proxy must inform MedStar Health immediately by contacting the MyChart Help Desk at 877-745-5656. MedStar Health reserves the right to revoke proxy access at any time for any reason.
Revisions and Changes
MedStar Health may revise the information on this website or otherwise change or update the website, including these Terms and Conditions of Use, without notice to you. You are encouraged periodically to read these Terms and Conditions of Use to see if there have been any changes that may affect you. Your continued use of the website will signify your continued agreement to these Terms and Conditions of Use as they may be revised.
Disclaimer of Liability
MEDSTAR HEALTH EXPRESSLY DISCLAIMS ANY AND ALL WARRANTIES CONCERNING MYCHART AND ITS USE BY YOU AND PROVIDES THE SAME ONLY ON AN “AS IS” AND “AS AVAILABLE” BASIS, WHETHER EXPRESS OR IMPLIED, STATUTORY OR OTHERWISE, INCLUDING ANY WARRANTIES OF MERCHANTABILITY, TITLE, NON-INFRINGEMENT AND/OR FITNESS FOR ANY PARTICULAR PURPOSE, AND WARRANTIES OF PERFORMANCE, AND ANY WARRANTY THAT MIGHT OTHERWISE ARISE FROM COURSE OF DEALING OR USAGE OF TRADE. Under no circumstances shall MedStar Health be liable for incidental, special, indirect, direct or consequential damages or loss of profits, interruption of business, or related expenses which may arise from your use of or inability to use MyChart and other services provided hereunder, including, but not limited to, those resulting from defects in software, communication lines, virtual private networks, the Internet, or your Internet service provider, access system computer hardware or software or other service or device that is used to access MyChart and/or documentation, or loss or inaccuracy of data of any kind, whether based on warranty, contract, tort, or any other legal theory, and whether or not MedStar Health is advised of the possibility of such damages.
IN ADDITION, MEDSTAR HEALTH DOES NOT GUARANTEE THAT USE OF MYCHART WILL BE FREE FROM UNAVAILABILITY OF INFORMATION, DOWNTIME, SERVICE DISRUPTIONS, INTENTIONAL OR INADVERTENT INTRUSIONS, VIRUSES OR WORMS OR OTHER MECHANISMS THAT MAY ATTACK YOUR COMPUTER OR OTHER ELECTRONIC DEVICE. YOU ACKNOWLEDGE THAT YOU ARE SOLELY RESPONSIBLE FOR ENSURING THAT YOUR COMPUTER HAS ADEQUATE PROTECTIONS AGAINST ANY OF THE FOREGOING. MedStar Health takes no responsibility for, and disclaims liability from, any results or consequences from your intentional or unintentional sharing of, provision of access to, or disclosure of your username, password, activation code or medical record information available through MyChart. You agree to hold harmless, indemnify, and defend at MedStar Health’s option, MedStar Health, its affiliates, and their respective officers, directors, employees, physicians, volunteers, successors and assigns from any claims, damages, losses, liabilities, judgments, (including claims of lost wages, revenue, or business) as well as all costs and expenses (including reasonable attorneys' fees and costs) arising out of a claim by a third party relating to your use of MyChart in violation of these Terms and Conditions of Use.
Content Ownership
The information and content contained in MyChart, including but not limited to medical information relating to you, is owned in whole or in part by MedStar Health and/or its affiliated entities. Other information contained in MyChart may be owned, in whole or in part, by unrelated entities for which MedStar Health may be a licensee. All content or information created by or owned by MedStar Health shall remain the property of MedStar Health and its affiliated entities. All content or information created by or owned by an unrelated entity shall remain the property of that unrelated entity. MyChart may contain links to third party websites for convenience. MedStar Health does not endorse or control the content of any third party websites unless otherwise stated, and we have no responsibility for the content or material contained on such websites. MyChart may also contain trademarks and other copy written materials or information that are solely owned by, and shall remain solely owned by, MedStar Health, its affiliated entities, or other unrelated rights holders. Subject to the terms of use herein, you may download and reprint copies of information relating to you and your health condition to share with your healthcare providers or payers for your care and treatment. Any other use of the materials is strictly prohibited without our prior written permission and the permission of the applicable rights holder(s).
Governing Law; Miscellaneous
These Terms and Conditions of Use will be governed by Maryland law without giving effect to any choice of law rules or principles. You agree that any action arising out of or relating to these Terms and Conditions of Use or MyChart shall be brought exclusively in the Maryland State Circuit Court sitting in Howard County, Maryland, or if that court does not have subject matter jurisdiction, then exclusively in the United States District Court for the District of Maryland, Baltimore Division, and you waive any jurisdictional, venue, or inconvenient forum objections. Any cause of action or claim you may have with respect to MedStar Health must be commenced within one (1) year after it arises, except to the extent such limitation is not enforceable. To the fullest extent permitted by law, each party to these Terms and Conditions of Use waives his or her right to a jury trial with respect to any dispute or other controversy arising from hereunder or your use of or access to MyChart. If any of the provisions of this Agreement are held to be not enforceable by a court or other tribunal of competent jurisdiction, then such provisions shall be limited or eliminated to the minimum extent necessary so that this Agreement shall otherwise remain in full force and effect.
These Terms and Conditions of Use constitute the entire agreement between MedStar Health and you relating to the access and use of MyChart and any of its services. Any representations by our representatives, information contained in another agreement, or any other statements or information presented within MyChart that is inconsistent with or conflicting with the these Terms and Conditions of Use is superseded by these Terms and Conditions of Use.

I understand my proxy access to MyChart may be terminated:
- If a request to terminate the access is submitted; or
- If I have requested access to my child’s MyChart and my child turns 13 years old or advises MedStar Health of his/her emancipated status or receives healthcare treatment that the law otherwise allows my child to withhold his/her medical status from me; or
- If access disputes cannot be resolved; or
- If the legal relationship between myself and the patient changes and no longer permits me the right to view his/her medical records; or
- If MedStar Health, in its reasonable discretion, that cause exists to terminate access.
Proxy Invite Terms and Conditions (Optional)
By clicking the “Send Invite” button, you will grant access to your online medical information to another person. We afford the same degree of confidentiality to medical information on MyChart as is given to medical information stored by MedStar Health, its providers, clinics, and other entities. By giving someone access to your medical information, you waive certain confidentiality protections.
MedStar Health providers, clinics, and other entities are committed to protecting the confidentiality of your medical information. MedStar Health uses firewalls, passwords, encryption, and audit trails to safeguard patient information. We identify the records released and note the date of access each time a user accesses MyChart.
If at any point in time you wish to revoke someone’s access to your patient records through MyChart, you may remove their access by using the MyChart Menu to open the Sharing Hub. Select the ‘A family member, close friend, or caretaker” option, then select “Manage ongoing access to your MyChart account.” You may then edit or revoke access to your patient records. You may also call MedStar Health’s helpdesk at 877-745-5656.
Hello World Terms and Conditions (Required)
Hello World delivers interactive SMS and self-service features through native text messaging, so patients with or without MedStar Health MyChart can take actions like paying a bill and confirming appointments.
When you sign up to receive messages from MedStar Health, you agree to receive messages related to your relationship with MedStar Health and its affiliated physicians and advanced practice providers. These messages may include updates related to your clinical visits, MedStar Health MyChart account, billing notifications, prescription reminders, care management, and other important healthcare information.
You may opt-out of receiving text and email messages by updating your communication preferences in MedStar Health MyChart or by clicking on the unsubscribe link in messages received from MedStar Health.
Wireless carriers are not liable for delayed or undelivered messages, and message and data rates may apply for any messages sent to you from MedStar Health and to MedStar Health from you. Message frequency may vary.
Messaging Disclaimers – Medical Advice (Required)
Please review this information before submitting a medical advice request through MyChart. Medical advice messages are intended for non-urgent health concerns and are reviewed by your care team during normal business hours. Responses may come from a member of your care team on behalf of your clinician, and if you are experiencing a medical emergency, call 911 or go to the nearest emergency department instead of using MyChart.
Self-Triage Terms and Conditions (Required)
Symptom Checker is designed for patients that are in relatively good health with common complaints, to guide you on generally applicable options to get evaluated and treated. If you have chronic medical conditions that may respond differently during an illness you should check with your doctor. If you have been told that you are at increased risk of infection or on immune suppression you should not use Symptom Checker and should call your doctor. If your symptoms last longer than 24-48 hours, please call your doctor.
You can revisit the recommendation information provided by Symptom Checker the next time you visit and utilize the Symptom Checker. The responses that you enter using the Symptom Checker will become part of your medical record. Symptom Checker should never be used for urgent health care matters. If you believe you are experiencing an urgent health condition, please contact your provider, go directly to the Emergency Department of a local hospital, or call 911.
Use of Symptom Checker constitutes acceptance of these Terms and Conditions of Use and is subject to the MyChart Terms of Use.
Proxy Access:
Symptom Checker may be used by a proxy on the patient’s behalf. Be sure you are in the correct patient’s account when using Symptom Checker because the information you provide will become part of the medical record.

E-Visit Terms and Conditions (Required)
What is an E-Visit?
An E-Visit is an online, message only medical visit. Instead of talking with a provider in real time, you answer questions about your symptoms and health history online. A member of your care team reviews your information and sends you a response, usually within 1–2 days. E-Visits are offered for your convenience and to save you time by providing care through MyChart. Use of the E-Visit service is subject to the MyChart Consolidated Terms and Conditions of Use that you previously accepted to access the MyChart services.
When should I use an E-Visit?
An E-Visit may be a good option when:
- You have a non-urgent medical concern such as Acne, Male hair loss, and/or erectile dysfunction.
- Your symptoms are mild or have been ongoing and do not require immediate attention.
- You have a condition or concern that can be evaluated based on your symptoms and health information.
- You would normally schedule with your primary care provider or visit an Urgent Care, but prefer to receive care from home.
An E-Visit can be a convenient alternative to an in-person appointment—you can complete it from home without waiting.
When should I NOT use an E-Visit?
E-Visits are not for urgent or emergency medical concerns.
It can take up to 2 days to receive a response.
If you need care sooner, On-Demand Virtual Urgent Care is available 24/7. You can also schedule an appointment with your primary care provider or visit one of our urgent care locations.
For any medical emergency, call 911 immediately.
What should I expect?
- Answer questions online. It typically takes 5-7 minutes to complete your E-Visit. You may be asked about your symptoms, medications, medical history, and insurance information.
- Submit your E-Visit. Please have your medications and health history available.
- A member of your care team reviews your information. You can expect a response within 1–2 days.
- Follow the care recommendations provided. Depending on your symptoms and the information you provide, your care team may recommend an in-person visit or another type of care if an E-Visit is not appropriate.
Will I be charged?
After you accept the terms and conditions, you may be asked to provide your credit card and insurance information. You will not be charged until your E-Visit is reviewed by our clinic staff. An authorization hold may temporarily appear on your card.
If you do not complete the E-Visit, you will not be charged. If you are redirected to another type of care, you may also not be charged.

MyChart Bedside Tablet Terms and Conditions (Required)
You may be offered MyChart Bedside to access some of your clinical information while you are admitted as an inpatient to a MedStar Health facility. MedStar Health is under no obligation to offer any patient the ability to use MyChart Bedside and may exercise its discretion in offering such use. Use of MyChart Bedside is free of charge to you as the patient. Some of the information in MyChart Bedside may only be available for a limited time after you are discharged, and not all past inpatient encounters will be available for viewing.
You may be offered the use of a MedStar Health-owned tablet to use while an inpatient to access MyChart Bedside. You will be able to access and use several applications (“apps”) on the tablet using your own personal account log-in. You will not be able to download any apps that have not been preloaded onto the tablet. By agreeing to use MyChart Bedside, you understand that the tablet provided to you is a loan. You agree not to take the tablet outside of your hospital unit and agree to take reasonable measures to protect the tablet from loss or damage. You may be responsible for any loss or damage to the tablet caused by you. At the end of your hospitalization, you agree to return the tablet to the docking station within your hospital room or to a member of your clinical team. All of your personal health information and any other data, including any log-in information, that was stored in the tablet will be erased once you are discharged from the hospital.

The information in MyChart Bedside may not fully reflect clinical data, such as the clinical staff assigned to your care team. If you have any questions, you should ask a clinical team member. MyChart Bedside should not be used as a replacement for necessary communication with your clinical care team. Any question you may have about your medical care or lab results should be directed to your care team. You should not rely on information provided by MyChart Bedside to make decisions.
MyChart Bedside has a feature called “Happening Soon”. You may enter personal events, but those will not be visible to the clinical staff involved in your care. Please note that entering this information is for your purposes only.
MedStar Health reserves the right to remove the tablet and/or discontinue your MyChart Bedside account or a portion thereof for any reason, including, but not limited to, abuse or mishandling of the application or tablet, interference with clinical care, behavioral issues, or failure to comply with these Terms of Use or MedStar Health’s policy. Such termination may occur without any notice to you. These actions are in addition to and not in lieu or limitation of any other right or remedy we may have available at law. Further, we shall not be liable to you or any third party for any such termination of discontinuance. The preceding Terms of Use will also apply to your use of MyChart Bedside. Any reference to MyChart in the following Terms of Use will include MyChart Bedside.
MyChart Bedside Tablet Proxy Disclaimer (Required)
If you are a proxy to an admitted patient using the MyChart Bedside application on a hospital owned tablet, the following terms and conditions apply to you. Both the use of the tablet to access your proxy subject’s account and the account itself are free of charge to you as the proxy to the patient. Upon discharge from the hospital, all of the patient’s health data, and any other data that was stored in the patient’s MyChart Bedside account will be cleared from the device you were using during the admission. Before the patient’s discharge, they will be given the opportunity to sign up for their permanent MyChart account, which they can access from their own computer at home or through the MyChart mobile application on their mobile device.
