Medstar Health Performs First Endoscopic Nipple-Sparing Mastectomy in Maryland

New minimally invasive procedure offers more options for breast cancer patients.

OLNEY, Md. — MedStar Health breast surgeon Jennifer Son, MD, has performed Maryland’s first endoscopic nipple-sparing mastectomy at MedStar Montgomery Medical Center, introducing a minimally invasive surgical option for select patients undergoing mastectomy for breast cancer.

The first patient to undergo the procedure in early September was a woman in her late 30s, and preserving her appearance was an important part of her treatment plan and recovery. Skin and nipple-sparing procedures can help create a more natural appearance after breast reconstruction.

During the procedure, the surgeon removes the breast tissue while preserving the outer skin, nipple, and areola whenever it is safe to do so. Using a small incision near the underarm, the surgeon inserts an endoscope, a thin tube with a tiny camera that provides a clear view inside the breast. Specialized instruments are then used to remove the tissue through the small opening. Breast reconstruction is performed during the same surgery, helping patients achieve a more natural appearance while avoiding additional operations.

Choosing the right patients for this procedure is important. The decision is based on each person's cancer diagnosis and individual needs to ensure the safest treatment and the best possible appearance and function after surgery.

According to Dr. Son, several factors help determine whether a patient is a candidate, including breast size and shape, how much the breast naturally sags, and the size and location of the tumor. In general, patients may be eligible if they have small to medium-sized breasts, little sagging, and a tumor that has not spread to the skin, chest wall, muscles, or nipple area.

“Breast cancer surgery is not one-size-fits-all,” said Dr. Son. “For appropriately selected patients, endoscopic nipple-sparing mastectomy gives us another option to consider when developing a surgical plan that takes into account their cancer, anatomy, reconstruction goals and preferences. Our priority is to provide safe, effective cancer care while also helping patients understand their options and feel confident in their surgical plan.”
